MEDICARE
HOW TO ENROLL IN AND SOME IMPORTANT NOTES


The first step to enrolling in Medicare is to download and fill out the Medicare enrolment form (MS004) with all the required information.


Please find the enrolment form at the link: https://www.servicesaustralia.gov.au/ms004


You can use this form to enroll in Medicare for the first time, re-enroll in Medicare, or enroll in Medicare for your child.


Then, you need to send your form and necessary supporting documents by post or email to the Medicare Enrolment Services. To help them process your request, please clearly write that it is for Medicare Enrolment in the subject line. The supporting documents for your enrolment do not need to be notarized.
Some people can enroll online using their myGov accounts.


There are 2 ways to fill out this form:

  • Type on your computer using Adobe Acrobat Reader or a similar program. You can download Adobe Acrobat Reader for free.
  • If you cannot complete the form by typing on the computer, you may print it out, fill it in by hand, and send it back to us following the instructions on the form.
